FOR THE LOVE OF OUR NATION CORP., (Love Our Nation) is a nonprofit 501c3 Florida Corporation. We are honored to share with your wonderful child the Reading and Math After School Homework Program, featuring Earth 2 Table (E2T) gardening program, Basic Etiquette, Financial Literacy, The Library, Golf with “The First Tee” Broward, Parades, Field Trips and other events that may be offered by our organization.

Love Our Nation Permission Form

Girls should start with their regular street shoe size to a half size larger. Boys should start a half size to a full size larger than their regular street shoe size. The shoes should feel snug with your toes flat and near the end of the shoe. The shoe will mold to your foot with wear. To ensure that the shoes are supportive of your foot and ankle, a snug fit is recommended.

Pickup Authorization

Other than than Parent(s), who is authorized to pick up your child daily, as well as from official Love Our Nation events (Must be 18 years of age or older.)

TERMS: By checking the "I Agree To The Terms" checkbox below, you agree and give permission for your child to participate in Love Our Nation programs from August 5, 2024 –  August 29, 2025 including, transportation, field trips, excursions, camps, events, and to any emergency relocations), and release/indemnify Love Our Nation, its staff and volunteers, from any liability claims arising from child’s participation.
CONSENT: Love Our Nation will photograph/film the program activities listed above which will be posted on our website as well as social media. By selecting "I Consent" in the checkbox  below,  you consent to allowing Love Our Nation to publish your child’s picture on the internet, social media sites, and all other public relation material associated with the organization or the Earth 2 Table program.

Volunteering is mandatory. You must fulfill your monthly obligation on order for your child / children to remain in the program.
